Don't know if this is the problem, but maybe give the game some more ram to use. Those settings can be found under Launcher Options -> Java Settings -> Memory.

xreliquary{1.2} [Reliquary] (Reliquary-1.2.200.jar) Unloaded->Constructed->Errored I found this in your crash report. To fix it all you have to do is delete the mod Reliquary or update it to a dev version.
