Alchemy bags randomly emptying and RM swords turning to DM?

On my tekkit server, I got two reports of their alchemy bags emptying, I personally doubt it and they're just trying for free stuff, but if it can happen, I'd like someone to tell me how to fix it.

Another problem is one person has a complaint that he made an RM sword, and when he disconnected, and returned, it was now DM. (Again, I bet its likely hes just trying for free items, but in case I'd like this fixed before my server gets bigger.)

I've seen alchemy bags disappearing but when a new one is crafted the items are still there.

No, I mean the items inside are disappearing, not the bag itself.
