Something is wrong with the server network, or your local network. Somewhere along the line the network is having problems, which makes you disconnect.

Someone in the server placed a wooden pipe, probably a griefer, because wooden pipes are bugged and WILL (in most cases) crash the server... try to whitelist the server, and tell people to not use wooden pipes and magically your problem is fixed.

You need to select "use development builds" in the launcher by clicking on the options button.

it's the screen resolution, the game/java/your graphics card doesn't like your screen, because the "Pixel format not accelerated". How to fix it? well all you need to do is un-tick LWJGL in the launcher options button. That should fix it.
