Ive deleted the 'Shatter' mod pack and the 'Mob dismemberment' mod pack, seems to have solved the problem!

i had this problem with other packs, try turning off your anti virus and firewall and run as administrator

I have the same issue! its so annoying! please post if you find a fix! Just to add, it looks to be Random mob body parts, for example i'm sure ive seen an upside down Skeleton head and partial neck aswell as a zombie leg/ arm, just as examples.
