Well, to be honest, I don't think there could be any other cause than an human error. Have you put it in a recycler while emptying your cobblestone? Have you melted it in a crucible? The possibilities are endless.

D: I can't believe that nobody suggested tesla coils! Place a pressure plate, a redstone under the block the plate is on, tesla coil aside the redstone, wire the tesla coil to a HV transformer with a redstone signal so the coil receives a EV signal. There, you can insta kill anyone who touches this plate with a zap of deadly electricity. I think it may even damage noticeably the charge of someone in quantum armor.

I'm not sure, but I think in game music can be found in the resources format, in the ogg. format. If you replace the original tracks with yours, i think it should work, as long as yours has the same name. I'm not sure what would happen with the length, all this is hypothetical.
