Well I would recommend asking on the Dimensional Doors MCF thread, assuming thats where the dev watches, but to take a guess, I'd say that whatever was causing serious issues in the world probably ate those other dimensions from the server files. Either that or you mangled the server config and somehow that's messed this up.

Please contact the modpack author if its not an official, comes with the launcher, pack. If it is a pack that came with the launcher, please post a full and proper bug report as opposed to commandeering another post.

I have absolutely no clue what you are talking about. Is this a custom modpack? Is this a individual mod? Is this a map? This a torrent of some homebrew movie? If the awnser is any of the above, you'll want to ask the person who made it, as this wouldn't be the place.

Because it doesn't care if you have minecraft on your computer. Its asking the mojang account if you've paid for a copy of the game, and mojang says no. Make sure your entering your mojang account details, and not your forum account details.

the pack will contain whatever they want. if you want ICBM with biomes a plenty and whatever else keeps your jimmies from getting rustled, you could make it yourself.
